投保须知：
1.入保年龄为15天至64周岁, 续保最高年龄至 75 周岁。Insured age is from 15 days to 64 years old; the maximum renewal age is 75 years old. 2.主被保险人的配偶或子女（投保时应未满 21 岁；为全日制在校学生的，年龄应未满 26 岁）可作为本合同的附属被保险人；0-17 岁子女只能以附属被保险人身份入保。The spouse or child of the main insured (child must under age 21; Fill-time student must be under age 26) can be regarded as the additional insured of this contract. Child aged form 0 to 17 only can be regarded as additional insured.
3.在保险期间内，被保险人应当在中国大陆居住合计180天或以上时间；During the period of insurance，the insured should reside in mainland China for a total of 180 days or more.
4.投保人或被保险人职业为律师、医疗机构从业人员、高风险职业从业者，需经核保确认后方可投保，否则不予承保。The insurance liability is subject to underwriting confirmation, if the policy holder or the insured occupation is a lawyer, a medical practitioner, or a high-risk professional.
5.事先授权Pre-Authorization 保险计划中有些特殊的治疗项目需要事先授权。客户在获得这些治疗前需事先确认该项治疗费用是否能得到理赔以及理赔标准是多少。Some special treatment items in the insurance plan require Pre-authorization. Before receiving these treatments, customers need to confirm in advance whether the treatment costs can be covered and what the compensation standard is. 需要事先授权的治疗项目包括：Treatment items that require Pre-authorization include:
（1）所有住院治疗都需要事先授权（包括生育/分娩的住院）；Hospitalization- Pre-Authorization is required for all locations. (Incl. Hospital stay for Maternity / Delivery);
（2）任何需要全麻下进行的门诊手术；化学治疗，放射治疗，血液或腹膜透析; Outpatient Surgeries requiring general anesthesia; Chemotherapy, radiation therapy, hemoperitoneal dialysis;
（3）购买或者使用非一次性耐用医疗设备，包括但不限于胰岛素泵及其配套器械；Purchase or rent disposable, durable medical equipment, including but not limited to insulin pumps and their associated equipment;
（4）紧急医疗转运； Emergency medical evacuation；
（5）牙科意外伤害修补治疗；Dental accidental injury repair treatment；
（6）每剂超过8,000 元的药剂或疫苗。 A drug or vaccine that costs more than ￥8,000 per dose.
（7）不在上述范围之内但保险期间内预期累计医疗费用超过人民币 40,000 元的任何治疗；Any treatment that does not comply with items 1 to 12 but has a cumulative medical cost of more than RMB 40,000 during the insurance period 客户在进行上述治疗前若未获得医疗服务中心书面许可回复，被保险人接受医疗的，保险人有权不承担保险责任。如遇紧急情况，被保险人只需在该紧急情况发生后 48 小时内通知即可If the client does not obtain written permission from the medical service center to reply before the above-mentioned treatment, and the insured receives medical treatment, the insurer has the right not to bear the insurance liability. In case of an emergency, the insured only needs to notify within 48 hours of the emergency
6.既往症Pre-existing Condition 在保险人对其保险责任生效前被保险人已就此接受诊断、医学咨询或治疗，或服用药物，或显现症状的任何疾病或损伤或精神疾病。Any Illness or Injury, physical or mental condition, for which an Insured Person received any diagnosis, medical advice or treatment, or had taken any prescribed drug, or where distinct symptoms were evident prior to the effective date.
